Financial Assistance Program

Eligibility

Cochran Memorial Hospital and Morton Rural Health Clinic offers reduced or no charge services for all emergency or other medically necessary care for individuals eligible under our Charity Care and Financial Assistance Policy, available below. The eligibility criteria is outlined in Section 2 of the County Indigent Health Care Program Handbook (CIHCP). Eligibility for Cochran County Indigent Healthcare is based on the County Indigent Health Care Program Monthly Income Standard as set forth in CIHCP and updated annually. The same forms are utilizied for charity care as set forth in the CIHCP for Indigent Health Care. Eligibility for charity care has been set at 200% of the Federal Poverty Guideline.



Guidelines for Eligibility

  1. Must be a resident of Cochran County.
  2. Not have any other source of payment or income such as insurance, governmental assistance or savings.
  3. Meet all count eligibility guidelines defined in the County Indigent Care Program Handbook (CICH).
  4. Provide proof of income and all resources.
  5. Complete the Application for Health Care Assistance and provide all information required by the Indigent Program.
  6. The most current year Federal Poverty Income Guidelines will be used to determine income eligibility.
  7. Application for Health Care Assistance and information to determine eligibility for the most current fiscal year are available upon request from the Cochran Memorial Hospital Indigent Program.
  8. For those who do not qualify for the County Indigent Care Program, Cochran Memorial Hospital offers a sliding scale payment plan based on income.
  9. You cannot qualify for the County Indigent Care Program in more than one county simultaneously.
